You'll need to make sure you're logged into the same account you requested the ride during which the item was lost. Select that ride from the list, then click Next and select what type item was lost, as well as fill out any additional information such as the best phone number you can be reached at and submit the form.
Depending on when exactly the item was lost, you either will be able contact the driver directly via the "Call" button, or you'll have to select the Contact Us button and leave a phone number for the Uber representative to call you back and put you in touch with the driver.
